It is an illustration about description of the portion “設欲求女便生端正有相之女宿植徳本衆人愛敬.”
It is a right half of the inside divided into right and left on account of a layout.
The title of an illustration is “a figure of Horizontal Hagi Minister-of-the-Right husband and wife
Chokoku-ji is filled.”
Description of the sutra is drawn in the first half.
If it translates quite roughly, a wonderful woman will become “If the Goddess of Mercy is believed in and wished, when you want a boy, a wonderful boy is born, and born when you want a woman.”
Moreover, it is written that it is not the same as fulfilling the wish which asks for a child to recommend the acolasia which is one of “Three poisons.”
It is because it is for praying the Kannon’s and accomplishing a child’s earning a world patch, and not wiping out an ancestral family name.
Since this is natural as a person, the acolasia is if different.
Then, the following is the free translation of an illustration portion.
“A reign of Emperor Shomu, old days and the 45th generation Emperor.
the Minister of the Right was worked Yokohagi Toyonari.
He was sad for a child not being born even if it turns 30 years old.
Then, it went to the Goddess of Mercy of 和州 Chokoku-ji to prayer together with all of husband and wife.
Inside was filled, 普門品 (Fumonbon) was read out and it prayed hard as long as 17 days.”

「設欲求女便生端正有相之女宿植徳本衆人愛敬」という部分の解説に関する挿絵です。レイアウトの都合上、左右に分けたうちの右半分です。挿絵のタイトルは「横萩右大臣夫婦 長谷寺参籠の図」です。
前半には経文の解説が描かれています。かなり大雑把に訳しますと「観音菩薩を信仰して願えば、男子が欲しい時にはすばらしい男子が生まれ、女子が欲しい時にはすばらしい女子が生まれる」となります。また、子を求める願いを叶えることと、「三毒」の一つである淫欲を勧めることは同じではないと書かれています。なぜなら、観音に念じて子を成すのは、世継ぎを儲けて先祖伝来の家名を絶やさないためだからです。これは人として当然のことなので、淫欲とは違うとのことです。
それでは、以下が挿絵部分の意訳です。
「その昔、45代天皇の聖武天皇の治世で右大臣を勤めた横萩豊成がいた。彼は、30歳を過ぎても子供が生まれないことに嘆いていた。そこで、夫婦揃って和州長谷寺の観世音へ祈願に向かった。17日も中に籠り、普門品（ふもんぼん）を読み上げ懸命に祈った」
